- Must include sales repās name and email
- Must be dated within 6 months of the application deadline
- Must be in Canadian dollars with tax (HST/PST/GST) included
- Only one quote per application (no mixed equipment categories)
- Quotes must match the request amount exactly
Online quotes and multiple vendor quotes will result in an incomplete application.

- Profit & loss statement
- Annual budget or audit
- Financials must show both revenue and expenses.
3. Equipment Inventory
If your organization owns apparatus or specialized gear, include a list (e.g., fire trucks, extrication tools, PPE). If not, submit a document confirming no applicable inventory.

Additional Notes
Preference is given to departments located within 96 km (60 miles) of a Firehouse Subs restaurant, but rural applicants are still encouraged to apply.
